CB Ajax File Field
The resulting file fields can be further configured to allow inline frontend Ajax powered editing (in addition to the profile updating method), file type filtering and other options.
This is a commercial add-on.

This CB Plugin (installed using the CB Plugin Manager and not the Joomla Installer) will add a File type option that you can use to create as many file fields as you want in your CB user profiles.
